Background Checks for BMET Interns
BMET students may be required to complete a Criminal Background check in order to participate in a clinical/internship with a hospital. PCC contracts with Pacific Screening for our BMET Clinicals/Internships.
If required by a hospital/clinical, students can request a background screening by Pacific Screening. Payment is the student's responsibility. Pacific Screening will send the results to George Knox, Cooperative Education Specialist. He will then forward the results to the student's assigned hospital.
How to Order a Background Check Report
In order to complete this transaction, students should:
- Complete the Screening Application and Payment forms available here. (Make sure the Requestor is listed as George Knox and the Company as Portland Community College - BMET.)
- Submit the forms, via FAX# 503-297-1904 ATTN: Dan Meister. Students may have the forms FAXed from the Jobs Office (CC 221).
- Complete the Consent to Release Information form available here. (Students can combine release for background check, drug screen and immunizations as needed.)
- Submit the Consent form directly to George Knox in CC 221, NOT the Business Office or Student Records. (E-mail or fax is OK.)
Once the screening report is received AND the Consent to Release Information form is received, George Knox will forward the results directly to the hospital contact listed on the Consent form. Students my request a copy of the results in person from George Knox.
Background Check Fees*
There are currently three different levels of criminal background check: 1) National Criminal Search and Oregon OJIN Search - $25; 2) OIG Check - $5; 3) Enhanced Criminal Search - $40. All students will get the standard Oregon search, with each additional level adding to to total fees. (If a student needs all three, the total would be $70.) Pacific Screening also charges a $3 service charge per transaction. It is the student's responsibility to determine which screening(s) their assigned hospital needs. Also, all fees are paid by the student.
